We accept most major insurance plans and are contracted with a wide range of providers, including Aetna, Alignment Health Plan, Amada, Anthem Blue Cross of California (including Pers Platinum), Blue Cross Blue Shield, Central Health Plan of California, Cigna, Health Net PPO, Humana, MultiPlan, Network by Design, Partners Health Direct, Prime Health Services, Sante, Traditional Medicare, TriWest, and UnitedHealthcare.
If you do not see your insurance listed, please don’t hesitate to reach out to our office. We are happy to verify your benefits and help you understand your coverage.Please bring a valid photo ID and your insurance card(s) to your first appointment. If you’ve completed your new patient paperwork in advance, please bring that as well.
We also recommend bringing a list of current medications, any relevant medical records or referrals, and a form of payment for co-pays or other costs.
Yes. To establish care with our practice, we require a referral from your primary care provider or another licensed healthcare professional.
The referral should include relevant medical records and a brief reason for your visit so our team can review your case and schedule you with the appropriate provider. Once we receive it, we’ll contact you to set up your appointment.
Please note that some insurance plans also require a referral for coverage. If you’re unsure whether your referral has been sent, feel free to contact our office for assistance.
Before your appointment, we will mail you a New Patient Packet that includes important information about your visit along with printed copies of our new patient forms.

Yes. Our office is located in a fully accessible building designed to accommodate patients with mobility needs. The facility offers ample accessible parking, step-free entryways with ramp access, and elevator service to our suite.
Once inside, hallways and treatment areas are wheelchair accessible to ensure ease of movement throughout the visit. If you require any additional assistance during your appointment, our team is happy to help. Please let us know in advance so we can best support your needs.
Our office is open Monday through Friday from 8:00 AM to 5:00 PM. We strive to provide convenient access to care and make ourselves available during standard weekday business hours.
